Registering at Kinmylies Medical Practice

If you wish to permanently register with the practice, please complete the two registration forms and bring them into the practice along with evidence of your identity and proof of address.

You will be offered an appointment with one of our nurses for a 'new patient health check'. You will be registered with the practice, but not with a specific doctor. Once registered you can see any of our doctors.
